Grant Forms

LDCC Grant Interest Form: This form should be submitted to the Office of Grants Development prior to ANY grant participation or collaboration to ensure efforts are not being duplicated and for prior administrative approval to proceed with the project.

LDCC Grant Proposal Approval Form: This form should be submitted to the Office of Grants Development - along with completed grant proposals and all attachments/forms - before submission. Before submitting any grant, administrative approval is required.
