*Sign Up form is below.
*The CONTRIBUTOR may include the student's parents, the spouse of the student's parent, or the student's spouse.
Students/*contributors will need the following at their appointment:

- Sign into your studentaid.gov account, prior to your appointment, to verify there are no sign-in problems.
- 2022 Taxes and W-2’s for student and contributor if applicable.
NOTE: if the student is classified as a dependent based on Federal guidelines, the contributor must create or have an existing FSA ID.
Tips for creating FSA IDs:
- Student FSA ID Username and contributor FSA IDs are created through www.studentaid.gov
- Student & contributor(s) can’t have the same FSA ID Username, email address, or phone number.
- All phone numbers and email addresses that are used must remain accessible/active through the FAFSA process. If your phone number or email address can’t be accessed when signing into studentaid.gov, a delay in submitting your FAFSA will occur.
